Post Discharge Clinic

Our Post-Discharge Clinic provides care for patients age 18 years and older. In addition to hospital follow‑up visits, providers will also be able to complete pre‑operative clearances and college sports physicals.

Services Provided:

• Post-hospital discharge follow-up for patients without a PCP
• Pre-operative clearance visits, as needed
