Chronic Care Management: Experience The Blessing Difference

Chronic Care Management allows you to connect with your healthcare providers more frequently over the phone or in person, making the care you receive more effective, quicker, and easier.

Chronic conditions, or chronic diseases, are medical diagnoses that are long-lasting. The word “chronic” is used when the disease or condition lasts for one year or more. Chronic diseases require frequent medical attention and are very common. Six out of ten adults in the United States have a chronic disease, and four out of ten adults have two or more chronic diseases.

Examples of Chronic Disease Diagnoses

Some of the most common chronic disease diagnoses include (but are not limited to):

  • Diabetes
  • Chronic Kidney Disease
  • Arthritis
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Heart Disease
  • Hypertension
  • Congestive Heart Failure
  • Hyperlipidemia
  • Asthma

Chronic Care Management Eligibility

To qualify for CCM enrollment you must:

  • Have 2 or more chronic conditions
  • Have Medicare Part B

Consent to participation in Chronic Care Management is a Requirement.

How the Program Works

You will work with our clinic team members on a regular basis, in between your regularly scheduled visits with your doctor, to create personalized care plans reflecting health goals important to you.

Topics you will likely discuss include lifestyle changes, reviewing medications and care instructions, introduction to community resources, addressing barriers preventing you from achieving your goals, and coordination of care.

At Blessing Health System, we believe that by using our entire team to care for our patients, we provide a more holistic and personalized approach to treatment of chronic conditions. Since Chronic Care Management can be conducted over the phone, the service provides patients with more frequent care and interactions with their health care team. It may reduce the number of trips to a medical office, as well a help decrease emergency room utilization and inpatient admissions.

  • Assistance with coordinating care between your doctors, pharmacy, testing centers, and hospitals
  • Phone check-ins between visits to keep you on track
  • Expert assistance with setting and meeting your health goals

Cost of Chronic Care Management

There is no enrollment fee for Chronic Care Management. Out of pocket, or co pay expense, for this program is minimal. Chronic Care Management is covered by Medicare – 80% covered with traditional 20% coinsurance. Some supplements, or Medigap plans, cover the coinsurance. Please talk with our care coordination team for an estimate of any potential out of pocket costs.

Recent studies show that Chronic Care Management services may decrease health care spending for patients with certain chronic conditions by nearly $30,000.
